Back Home (SSVF) Program Fact Sheet

  • The Back Home Supportive Services for Veteran Families (SSVF) Program is a Housing-First program designed to deliver wrap around case management, temporary financial assistance, and other supportive services to veterans and their families. Please call 303-6701796 on Monday from 10:00 am to 6:00 pm or Tuesday, Wednesday and Thursday from 10:00 am to 4:00 pm to make an appointment. Case Managers are onsite at EChO every Tuesday from 10:30-1:30.

Eligibility

  • A Veteran must be a member of your household. Definition of 'Veteran’: any discharge other than dishonorable, with at least 1 day of active duty service (basic training counts as active duty). However, if you served in the Guard or Reserves, you must have been called to active duty under Federal/Presidential orders.

  • Your household must have a combined income of less than 50% of the Area Median Income (AMI)

  • You must either be:
Currently homeless (to include transitional housing program such as the Domiciliary, GPD Programs, or vouchered motel rooms)
    OR
Currently residing in permanent housing (to include “couch surfing” or at the residence of family/friends and staying in motels that the client has paid for) but are at imminent risk of becoming homeless.

Documentation

  • Some documentation/information that is helpful to have on hand during your screening:
Verification of Veteran Status (VAMC ID Card, VA Statement of Service, HINQ, DD214 Member 4 Copy)
Proof of income (bank statements, benefit award letters, pay stubs, etc.)
Social Security Numbers for all members of the Veteran household

Note: This is a short-term housing stability program, NOT an emergency financial assistance program.
Note: You can still be screened for eligibility without these documents on hand, but they will be required prior to program entry.
Note: There are fewer funds available for those at risk of becoming homeless, and the requirements for program entry are more difficult to meet.
